I know this off hand has no spirit on it but Im thinking of getting this as Resto druid. My only other option is either Chalice of Searing Light from 25 man TOC or the Antique Cornerstone Grimoire from Onxyia. Since Im mainly a 10 man raider I doubt I'll see the one from toc 25 and the onxyia one will seriously lower my haste which I need more than ever since 3.3. I just hope it will still do the job as a offhand for me until I can start running ICC raids.

No dps caster favors spirit over SP, haste, critical strike chance, it is a bonus stat enhanced with talents or glyphs.SP benefit from spirit gained through talents and glyphs require certain conditions, and in practice, the ratio of mentioned buff is very tiny especially for a shadowpriest, which makes it less important. In short if there is a mage, lock, shadowpriest and a disc priest in a group, all will roll and have equal right for need unless they have this item or better.

This is one of the better offhands that is somewhat easy to obtain for non-25-man-raiders. However, if you're still struggling to find a decent offhand you can always go to AH and buy the Faces of Doom, which is a surprisingly good alternative for healers in need of spirit. Don't let the level requirement scare you away - way too many people stare themselves blind on iLvL and rarity... I personally had it on my disc/holy priest until I got the heart.

Got this one today after running the instance allmost everyday since the launch of it.
Pretty much BIS for a disc priest who doesn't have the opportunity to run raids. This has everything you want: int, crit, haste and sp.

If you're a 10-man raider, the healing off-hand options are almost non-existent. Even in 10-man ICC. This item is about the best you can do.

The model itself is not too great to look at but the crit and haste combination is quite nice. I currently still have the il200 emblem of heroism offhand and really need to gain more haste for arcane and this according to RAWR is 9th from the top and is > then toc 25s mystifying charm for me due to haste. Granted it's marginal how much of an upgrade it is from that but I am thinking it's still easier to obtain lol.

Alright. I'll say it. If you're a HPal looking for an upgrade from the badge shield or the heroic AN shield, this item is amazingly itemized. Minus the lack of armor, of course. Don't be afraid to pick it up if clothie considers it a "sidegrade."

This sadly seems to be one of the only reasonable choices for elemental shaman who aren't able to raid much in terms of offhand/shields. Everything else has mp5 on it, which we really don't look for in gear.

So, I apologize to all you clothies and druids out there who are getting outrolled by us, but honestly it's all we have outside of ToC 25 and ICC.

This is the best OFF-HAND drop outside of raids for holy pallys, ele shamans and resto shamans, plus the standard users of off-hand items.

The best SHIELD for these 3 specs is the Protector of Frigid Souls from Pit of Saron regular.

Of note, a shield can be enchanted while an off-hand can not. So essentially your looking at; mp5 and higher int (assuming the enchant) vs haste and higher crit

I think it's worth noting that, although the item does look like any kind of shriveled heart, it emanates pretty red swirlies. I don't know about you, but that's what I look for in a good offhand.

A good step up from your Ward of the Violet Citadel, mages (and other classes). Unfortunately, it requires a few runs of H HoR and a little bit of luck.

18 runs so far... only seen the heart once and lost the roll. Though if you want the Fossilized Ammonite Choker join my group because that's dropped at least 5 times now.

Blizzard, please increase the drop rate on this heart! Not many off-hands available without hit on them and the drop rate is terrible on this item.

14 runs so far and I have yet to see this damn thing drop. Blizz should add code that increases the drop rate of items that have yet to drop for your character when it starts to become an obsence amount of runs with no drop.

Healers:
Priest (Discipline or Holy) - Surgeon's Needle + Shriveled Heart of course. (If hit capped then go with this for Shadow Priest)

DPS:
Affliction Warlocks - Seethe + Shriveled Heart (If hit capped then go with this for all Mage specs & the other two specs for Warlocks)
All Mage specs, Shadow Priest & the other two specs for Warlocks - Engraved Gargoyle Femur (If not hit capped)

I've noticed a lot of gear in HoR appears to be old world stuff.

I just picked up a robe, and it's the exact same as Runecloth Robes. :(

Just looking at the drops there are no Cloth Robes that drop from HoR, and the leather ones don't look like those at all.

Planning on using this with the rdps mace from the Quel'dalar chain. Compared it with the Mighty Spellpower buff, and although you lose a little intellect (I think), it's a very good upgrade from Engraved Gargoyle Femur. Looking forward to getting this :) You could also add the 22 intellect enchant, and still have far superior spellpower, but I wouldn't worry about it. The combo is superior in haste, SP, crit, and stamina.

After 34 runs (yes I counted) and only seeing this drop twice, and losing the roll twice, I finally gave up on ever getting hold of this bastard thing.

My life is ever so slightly better now I don't have to heal HoR heroic every day.

Seems like since 4.0.1 this is the best non-raiders offhand available for PVE mages.... Accursed spine became useless because of the spirit nerfs

Good grief the times i had to run HoR HC to get this for my warlock! must've ran atleast +30 times... And to add the times i lost Seethe over to a mage or a paladin.... still sends shivers down my spine. But alas a fair combination. Used it until i got Abracadaver from from Festergut.
