You get Mr. Pinchy https://www.wowhead.com/tbc/item=27388 from fishing from highland mixed schools. Mr. Pinchy then grants you 3 wishes... one of which can be the Furious Mr. Pinchy!

Just watched some gnome lock in Shatt summon this guy, and it tried to kill him... is this a random chance of you bringing out a raging lvl 70 Mr. Pinchy every time you call him out?

One possible location that he can be "caught" at is Lake Ere'noru in Terokkar Forest @59,61 but you have to have a flying mount to get there
